You're right, she did do that a lot. Ir's especially bad when you jump, 'cuz then the heels may never go down. One girl at my studio was out for 2 months because of an injury relating to that. It's best to fix it at the bar.

on the way down from releve, the heels must make contact with the floor before you begin to execute plie, it a common mistake :)
